The Quality Assessment Questionnaire is being developed by the Quality Working Commission to serve as the QP’s core assessment instrument. An important step in creating the Questionnaire was testing a pilot version in order to identify where improvements could be made. The pilot phase was conducted with the generous assistance of the following ANADO Members:
- Anti-Doping Authority of The Netherlands
- Caribbean RADO
- Japan Anti-Doping Agency
- Korea Anti-Doping Agency
- National Anti-Doping Agency of Romania
- Swedish Sports Confederation
The Phase 1 Questionnaire consists of 5 sections:
- Overview Information
- Test Planning
- Sample Collection Personnel
- Sample Collection
- Results Management
Members are invited to provide comments on the draft Phase 1 Questionnaire by February 15, 2010. The draft Questionnaire can be accessed as a read-only document using the following link:

The link to the final version of the Phase 1 Questionnaire will be on this page no later then March 31, 2010 for completing online using secure password access. At this time we will only have capacity for the Questionnaire to be completed in English by ANADO NADO and RADO Members. However, if supporting documents are requested and are not available in English, the documents will be able to be submitted without translation.
The Questionnaire will be able to be confidentially and securely completed all at once or in multiple sessions, and then submitted confidentially and securely online. The online submissions will go to the Chair of the Quality Assessment Panel who will initiate the quality assessment process, and also to ANADO’s Quality Program Manager who maintains the QP’s master files. |
